Yep its a 6 mana combo with . . . unsummon. . . or a 5 mana combo with slaughter pact. However does it even work?! Probably not - but it's fun to think about.
The playtest text for Form of the Squirrel, iirc, was something like "As Form of the Squirrel comes into play, put a 1/1 green Squirrel creature token into play. ... That is you." If that was even too much for Unhinged, then this is definitely too much for a normal black-bordered card.

Flavor-wise, it's been done with personal incarnation.
It would be less problematic rules wise to just make a sleeper agent creature that causes makes the player lose the game if it leaves the battlefield.

This card does not work in the present rules, but could be printed if additional rules were made for it.
For flavour reasons, I would expect the card to have a clause akin to Form of the Dragon's 'your life total is refreshed to 5 each turn'.
Of particular note: Do you want the player to be able to be attacked?
