It should be somehow different from Zombify, otherwise its just plain worser (and Zombify is just "fair"). Recurring Nightmare is something thats a lot like your card, just million times better (a real bomb actual).
It could easily be something more like this:

So this way you trade something "big" for multiple small stuff, different enough to Zombify.

With the reduced cost you can get a very fast reanimate effect, but its still hard to get a turn 1 creature followed by a reanimate (as reanimator normally don't play small creatures at all).
However, make it somehow diverse from Zombify / Recurring Nightmare.

Better as the card gets explosive, but 5 mana is still a lot as you might just change the creatures you sacrifice with your grave over and over.

Its just me as i hate cumulative upkeep with "manacosts" , the non-manacost option is much more attractive as it feels less like a way to limit the card and more that you use the age counters as a real mechanic to work with (rather than a pure drawback).
Idea is that the card is either a slow trip Damnation if you want it or it gets stuff back for you, both should be good and flexible enough.

Graveshift Ritual is worse than Zombify. I suggest making it an instant. At 4 mana, it is probably fine with only that bonus, but if you make it cost less or something, I would suggest making the sacrifice a creature an additional cost. As it is now, you can play it when you have no creatures without sacrificing anything, which is pretty cool and not that overpowered at 4 mana.
Shambling Forces seems unnecessarily complicated. You'll end up sacrificing X creatures each turn and reviving X creatures, but none of those can attack since they just came into play. It merely seems like a way to abuse comes into play abilities. I would suggest this:

You can still swap your creatures with those in your graveyard, but this won't stop your army in it's tracks after 2 turns. On the other hand, you only get to keep an extra guy when you sac it, unlike X+1 guys when you sac it. The upkeep is optional, so you refuse to pay this turn, but still revive X+1 creatures from it's trigger this turn, using last known information.
