DC Comics is rebooting "rebirthing" their universe again, and as much as these events normally cause a great rolling of the eyes, I have to say, Geoff Johns got this one off to an amazing start. Did anybody else's heart skip a beat when they saw that final panel?

So what went so wrong with the new 52 that they need to ditch it so soon?
I'm not sure this qualifies as "so soon". For better or for worse, DC does this fairly regularly. Infinite Crisis was in 2006, Flashpoint was in 2011, and now here we are in 2016. Business-wise, the New 52 was successful at increasing sales. But I do think you're right that things went wrong. There seems to have been some general fan discontent with the heavy (though not total) continuity reboot. And on the technical side, DC was trying to juggle 52 series and didn't do it very well. Inconsistent and arbitrary editorial control led to high creative team turnover on a number of series. That's not to say that all New 52 books are trash -- Batman and Wonder Woman in particular stand out as excellent. But there seems to have been growing demand for a return to the "real" DC Universe, and Rebirth is delivering in spades... with a twist that I don't think anybody saw coming.

I don't know where this mentality that DC is constantly rebooting comes from. DC has had only reboots in it's entire run. Crisis on Infinite Earths (85-86). Flashpoint (2011). And now Rebirth and Rebirth IS NOT a reboot. It is taking parts of the New 52 and remerging them with the "real" DC Universe.
Infinite Crisis and Final Crisis were just arcing events, but not reboots.

Okay, technically true, but you can't deny that they're pretty reboot-adjacent.
