1. I don't see any reason for this to be flip-card related.
2. My suggestion is to make it a reverse Living Weapon, with unique tokens:
Furnished N [Keyword] (This creature enters the battlefield equipped to a created Equipment token with Equip 1 and "Equipped creature gets +N/+N and [keyword].")
(Note: I'm not sure on the wording for it entering play simultaneously with a created equipment token...)

Make no mistake; yours has more design space. At the cost of more expensive printing and less rigidity. Of course, yours could just as easily create non-flip convoluted equipment as need be (enters the battlefield or leaves the battlefield), but I think the limits of Furnished make the card design much simpler to understand in limited.
Note that Furnish is french vanilla (albeit with nested keywords). Pull an Amonkhet with unique equipment for each creature.
This creates an interesting environment where players have lots of choices - do I reequip to make one evasion creature faster? Am I more willing to trade since I have something left over while the other player doesn't?
Heck, you could even do a legendary version:
[Name] Memento N - [keyword] (This creature enters the battlefield equipped to a created legendary equipment token named [Name] with Equip 2 and "Equipped creature gets +N/+N and [keyword].")
You stick memento on your legendary creatures and you're golden. Lots of flavor here.

[Name] Memento - +X/+Y - [keyword] (This creature enters the battlefield equipped to a created legendary equipment token named [Name] with Equip 2 and "Equipped creature gets +X/+Y and [keyword]")

This lets you do your slagwurm armor cards easily enough.

This concept was explored briefly with Harvest Hand/Scrounged Scythe, I see no reason why it can't be used again.
entombedhydra's version is MUCH more complicated in practice and a bookkeeping nightmare with such a wide variety of different specific tokens. Living Weapon works because all of the Germ tokens are identical. If you want to do it in reverse, making all the equipment tokens identical, or at least very limited in variety, is a must.
I'd be okay with Amonkhet-style tokens weapons 1 unique for each legendary creatures, but you're right Sanetali. It's probably best to produce a set number of tokens per set.
For the first set, I'd recommend these be used on multiple creatures:
Furnished +1/+1
Furnished +2/+2
and then a few +1/+1 + (color relevant keywords) (First Strike, and maybe vigilance?)
That'd be 4 tokens for about 8-10 commons and uncommons, and maybe a unique token for a legendary rare. Mind you, I'd hesitate to produce token creatures for the set in question.

I like this idea but I'm not super pumped by your rare, which IMO has too many moving parts and too many set-specific abilities.
